<p>Many people in the United States have a cursory knowledge of substance abuse therapy, but the popular media&#8217;s portrayal of drug rehab does not depict the average addict&#8217;s treatment. For the most part, only recovering addicts and their families fully understand the different types of treatment programs available in America. Every program has a specific purpose, but they all use similar therapeutic practices to illicit positive lifestyle changes from drug abusers. The most intense and longest-lasting of these plans is the residential inpatient program, a thirty-to-ninety day stay at a rehabilitation facility with fifty or more hours of intensive therapies per week. Unfortunately, most people do not understand the nature of these therapies and how they help addicts overcome their compulsions. It is important for laypeople to know what to expect during inpatient programs should they ever fall victim to the disease of addiction.</p>
<p>The one to three-month stay at a rehabilitation center is highly structured and supervised. Clinicians create an environment of isolation from compromising situations which would put addicts at risk of relapse. This isolation can be critical, and many healthcare professionals agree that the first thirty days of sobriety are the most important in breaking bad habits and dealing with negative thought patterns. For some patients, even longer periods of isolation are necessary to solidify positive lifestyle changes and develop strategies for coping with the inevitable real-world pressures to use drugs.</p>

<p>Another name for bipolar disorder is manic depression and it is often debilitating to those that are living with it. This does not include the impact that it has on the people around them that love them. It is important to know that there are two types of bipolar disorders. The first is bipolar disorder I which has moments of euphoria and moments of mania that are then accompanied by feelings of depression. Meaning that there are times when the person feels really good and times that the person feels really bad. Bipolar disorder II has alternating moments of hypomania and depression; this is the less severe of the two types. </p>
<p>The total number of people afflicted with a bipolar disorder is a little over two million, in the U.S. alone and the age range starts as young as 6 years old. With such a disparity in age, anyone can understand why finding a natural way to treat such a severe disorder naturally is important. You don&#8217;t want to start your child on a medication that is suppose to help them only to cause them more health problems down the road. The good news is, you don&#8217;t have to. There are effective natural treatment options. </p>

<p>Multiple factors are responsible for the cause of bipolar health disorder. Chemicals called neurotransmitters control the functions of the brain and any imbalance in any of these neurotransmitters, mainly the norepinephrine, may lead to bipolar health disorder. When norepinephrine levels are below normal, a person is said to experience depression and when the norepinephrine levels shoot up, a person experiences mania. Serotonin and dopamine are also norepinephrines that are believed to be responsible for bipolar health disorder.</p>
<p>The genetic component too plays a significant role and any family member with bipolardisorder can put other members at risk. Identical twin of a person suffering from bipolardisorder is at the highest risk for contracting the disease. However, genes alone are not responsible for the disease. Stress of any kind is required to trigger the onset of bipolardisorder. Environmental and genetic factors are the main reasons for bipolardisorder.</p>

<p><b>An Inpatient of Outpatient Treatment Center</b></p>
<p>When looking for a drug treatment center for drug detox or help with a drug problem, first decide on the level of treatment you need. Going into a hospital or facility is both expensive and traumatic for most people; it only begins the process of recovery. These days, more drug treatment is being done on an outpatient basis, and this makes a lot of sense because treatment of addiction is a long-term process.</p>
<p>Don&#8217;t go blindly to any one place for help. Your personal physician is a great place to start and can help you decide whether you need to be admitted to the hospital. Physicians have good contacts in their community and know who to go to and places to avoid. Their experience with therapists and their knowledge of psychiatrists in the area can make a difference in your decision. Avoiding hospitalization is not always the correct path, but doing so will save thousands of dollars and avoid having to miss work or other obligations.</p>

<p><b>Your Family Doctor Treats Addiction Too</b></p>
<p>Family docs are great, my wife is one. They have a happy natures and are willing to go out of their way to help their patients. There are now a variety of addiction treatments available in a physician&#8217;s office for the treatment of addiction.</p>
<p>We now have both pills and injections to treat alcoholism that can be given in the clinic. Some patients can receive medications at home to wean them from alcohol. We have buprenorphine for narcotic addiction. Some patients actually do quite well seeing their Family Doctor for very brief therapy sessions without the need of further counseling or treatment.</p>
<p><b>State Treatment Centers</b></p>
<p>Before contacting you personal doctor, or if you don&#8217;t have a Family Physician, call you State Mental Health Authority. They can be found in the phone book or online in a directory under &#8220;Mental Health, or Department of Human Services, or Office of Addictions, or Health and Family Services.&#8221; After finding out what is available in your area, discuss it with you own physician.</p>
<p>People would be amazed at the amount of help their individual States can provide. They usually have separate treatment tracts for addiction, mental health (depression, schizophrenia, bipolar illnesses), and mental retardation. The State offices will connect patients with local services, and the local offices will often do free screenings. They often work on sliding scales and are far less expensive than private services. It has been my experience that one can get good treatment in this manner if they show they are willing to attend all appointments.</p>
<p>The biggest downside to Local Mental Health Departments is they can take up to several months to get an appointment with their regular psychiatrist (and not just an emergency screener). When finances are a factor, I will usually see a patient the first few visits while working to get them in to the local Mental Health Departments.</p>
<p><b>Private Treatment Centers</b></p>
<p>Private Treatment Centers are usually expensive, and they have the perception of more privacy than State Facilities. These facilities are usually well advertised and easy to find. The services provided have nicer surroundings and the patients who go there usually have insurance or are working.</p>
<p>The main advantages of private treatment centers include their expanded services and improved level of training of staff. There may be more aftercare groups and more convenient hours. I&#8217;ve always heard that talent goes where there is money, and this is no different. It is not always the case, but more often than not, it is.</p>
<p><b>Free Treatment (or almost free)</b></p>
<p>If you addiction is alcohol, go to Alcoholics Anonymous. AA has a long history and works as well, or better than anything out there. It is readily available an usually even small towns have several meetings per day. Did I mention they were free? I only tell patients two things: 1. Try out two or three different AA groups because each group is very different from the next. 2. Get a sponsor with at least 5 years sobriety, preferably 10 years sobriety. In my experience, alcoholics with under 5 years sobriety usually don&#8217;t have the calmness, clarity, and experience to help newer members as sponsors.</p>
<p>If you addiction is other drugs, Narcotics Anonymous can be found the same as AA. I have found the quality of NA meetings not to be as good as AA meetings, so finding good sponsors is even more important. Consider avoiding meetings where there are few individuals with less than a year sobriety.</p>
<p>For those not interested in 12 step programs, I should note that being involved with you local Church and receiving pastoral counseling works for quite a few people. For those not religiously inclined there is Ration Recovery.</p>
<p><b>In Summary</b></p>
<p>There are many places to find treatment for addiction: from you Family Physician, to State Mental Health Agencies. There are expensive private facilities and free meetings around town. The key is to remember that addiction is a chronic illness. It will require ongoing treatment. One hospitalization or meeting will not fix addiction. Make sure you review you options with your Family Physician because they can treat addiction and help you make the best decision for you situation.

<p>For many years, a controversy has existed among health experts on the efficacy of using antidepressants on people with bipolar disease. The common fear is that using antidepressants can cause what is known as rapid cycling.</p>
<p>This is a condition in which the mood of a patient can switch from mania to major depression and back again &#8211; all in a matter of hours or days. And, many health practitioners believe that this actually hinders a patient&#8217;s treatment by making the disease more difficult to treat with more standard medications such as mood stabilizers.</p>
<p><span id="more-3325"></span></p>
<p>Others who treat manic-depression, simply doubt the general effectiveness of using antidepressants to treat bipolar depression. It is well known that the use of these medicines can produce &#8216;switching&#8217;. In other words, after taking an anti-depressant, a patient may become more alert, active, and less depressed.</p>
<p>When researchers looked into the phenomenon further, however, it was shown that, in many cases, the change in behavior and attitude was simply due to the person &#8216;switching&#8217; to the manic phase of his disease. The unanswered question, however, is exactly how often this occurs. Many who don&#8217;t believe in the use of antidepressants to treat manic-depressive behaviour believe that it happens much more than commonly realized.</p>
<p>The studies, to date, have been inconclusive. Partially because there simply have not been enough studies that directly address the effect of antidepressant medications on manic-depressives. And most of the studies that have been performed, haven&#8217;t effectively isolated effects of the various medicines involve.</p>
<p>For example, one study recently concluded that antidepressants, when used with mood stabilizers, had no discernible effect on the incidences of switching when compared to patients who were only given mood stabilizers. But, the study did not focus on the question of switching in those who only were given anti-depressants.</p>
<p>All of this begs the question of why use antidepressants at all in the treatment of bipolar. The answer, of course, is that some patients &#8211; albeit a small percentage &#8211; have been proven to do very well when given antidepressants.</p>
<p>The real question, however, is &#8211; given all the potential problems that antidepressants can cause &#8211; how large is this group of people that actually benefit by using them. But, the further question is, how can these people be identified? And, that, is a question that science has not yet answered.</p>
<p>Until the question is answered, however, some of the more conservative experts recommend that the patient first be treated without the use of antidepressants. And, only if they are non-responsive to these alternative forms of treatment, should they be treated with antidepressants.

<p>Depersonalization disorder is a dissociative disorder to which many folk can pertain. Depersonalization disorder is sometimes called &#8220;depersonalization neurosis. &#8221; Occasional moments of depersonalization are natural, but relentless or repeated feelings are not. Brief periods of depersonalization are notably caused by stress, a lack of sleep, or a combination. Depersonalization disorder is often associated as a comorbid disorder of anxiety disorders, panic disorders, clinical depression, and bipolar disorder. Some of the more common factors that exacerbate dissociative symptoms are negative affects, stress, subjective threatening social interaction, and unfamiliar environments. Factors that tend to diminish symptoms are comforting interpersonal interactions, intense physical or emotional stimulation, and relaxation. An episode of depersonalization disorder can be as brief as a few seconds or continue for several years.</p>
<p>Depersonalization becomes a disorder when the dissociation interferes with the cultural and occupational functions needed to mundane life. Often a victim of depersonalization disorder feels as if he or she is going crazy, though this is nearly never the lawsuit. Depersonalization disorder has been associated with childhood interpersonal trauma. Emotional abuse is a significant predictor of depersonalization disorder and depersonalization symptoms. The most common immediate precipitants of the disorder are severe stress, depression and panic, high grade marijuana and hallucinogen ingestion. Depersonalization can result in very high anxiety levels, which further increase these perceptions. Depersonalization disorder is not directly caused by a general medical condition or by substance use, including medications and drugs of abuse.</p>
<p><span id="more-3317"></span></p>
<p>The heart symptom of depersonalization disorder is the personal experience of unreality. Anxiety can aggravate depersonalization symptoms. The symptoms include a sense of mechanization, feeling a disconnection from one&#8217;s system, and trouble relating oneself to world. In addition, depersonalization disorder can cause anxiety since the person feels abnormal and uneasy at the loss of their sense of self. These experiences may cause a person to feel uneasy or anxious since they strike at the core of a person&#8217;s identity. Depersonalization is the third most common psychological experience, after feelings of anxiety and feelings of depression, and often occurs after life threatening experiences, such as accidents, assault, or serious illness or injury. These symptoms can interfere with a personâ€™s general functioning, including social and work activities, and relationships.</p>
<p>The diagnosis of Depersonalization disorder can be made with the use of various interviews and scales. Treatment is dependent on the underlying cause. To date, no treatment recommendations or guidelines for depersonalization disorder have been established. Treatment usually is needed only when the disorder is lasting or recurrent, or if the symptoms are particularly distressing to the person. A variety of psychotherapeutic techniques has been used to treat depersonalization disorder. Clinical pharmacotherapy research continues to explore a number of possible options. Treatment of chronic depersonalization is considered in depersonalization disorder. Relaxation techniques have been reported to be a beneficial adjunctive treatment for persons diagnosed with depersonalization disorder, particularly for those who are worried about their sanity.

Depression is prevalent among all age groups, in almost all walks of life.</p>
<p>Persons of any age&#8211;children or adults, may develop depression symptoms. Even minor stress events can stir up depression symptoms depending on the personality type. Symptoms such as intense sadness, loss of interest or pleasure in normal activities, sleep disturbances or oversleeping, change in appetite and decreased energy level; feelings of helplessness and thoughts of suicide are sequels to stress induced depression.</p>

<p>Causes of Depression</p>
<p>Some types of depression run in families, indicating that a biological vulnerability to depression can be inherited. This seems to be the case, especially with bipolar disorder. Families in which members of each generation develop bipolar disorder have been studied. The investigators found that those with the illness have a somewhat different genetic makeup than those who do not become ill. However, the reverse is not true. That is, not everybody with the genetic makeup that causes vulnerability to bipolar disorder will develop the illness. Socioeconomically disadvantaged groups have higher rates of depression compared to their advantaged counterparts. Immigrants to the United States may be more vulnerable to developing depression, particularly when isolated by language.</p>
<p>Regardless of ethnicity, men appear to be particularly sensitive to the depressive effects of unemployment, divorce, low socioeconomic status, and having few good ways to cope with stress. Women who have been the victim of physical, emotional, or sexual abuse, either as a child or perpetrated by a romantic partner are vulnerable to developing a depressive disorder as well. Men who engage in sex with other men seem to be particularly vulnerable to depression when they have no domestic partner, do not identify themselves as homosexual, or have been the victim of multiple episodes of antigay violence. However, it seems that men and women have similar risk factors for depression for the most part.</p>

<p>Bipolar disorder is the given name to describe a set of &#8216;mood swing&#8217; conditions that is felt by a person. The most severe form of it is called &#8216;manic depression&#8217;. This disorder may affect both sexes. However, women who have it will usually have more episodes of depression, while men are more prone to begin with manic episodes. Doctors stated that the disorder has no single cause. It has been shown that some people are genetically prone to it. However, not everyone with hereditary liability develops it. <p>Bipolar disorder can be very difficult to diagnose. In fact, many people may go for many years without being diagnosed. Bipolar can not be found through any laboratory testing. The only way to detect bipolar disorder is by having your medical provider use a questionnaire called the MDQ. The checklist can only be used and given the right diagnosis by a medical provider.</p>

<p>To treat your bipolar disorder, they would offer first specific exercise that is tailored to returning the balance that you have lost, and then they would recommend that you try to identify the trigger that set off the disease and learn to deal with it both by avoidance and confrontation.</p>

<p>Bipolar disorder is a very serious condition. Patients can suffer from terrible mood swings, which may lead to suicidal tendencies. Beyond that, there are times that it is extremely difficult to relate with them. Taking care of them can become frustrating and even irritating. </p>
<p>You should find a psychologist or someone to help you with your bipolar situation in a hurry the moment you even suspect that you may suffer from it. 			<content:encoded><![CDATA[<p>For those who have bipolar, depression, or some other mental illness, a holiday weekend is not always a cause for celebration. While the rest of the country goes looney with the joy of being sprung from work for an extra day, a person with a mental illness may be dragged down with the burden of being obligated to have fun. When we are expected to be happy simply because the calendar says that it is time to have fun, it can be very easy to become depressed about being depressed.</p>
<p>I have bipolar disorder and I have used this plan to survive holidays:</p>
<p><span id="more-3099"></span></p>
<p>1.  <strong>Do not burden yourself with the demand that you must have fun.</strong> You cannot flip the &#8220;Im happy&#8221; switch just because it is Labor Day Weekend. Sure, it is wonderful to attempt to do things you enjoy and have a good time. But if you are just not in the mood for fun, then it is even worse to feel guilty for not yukking it up with everybody else. (In fact, you are not alone. Not everybody is having ball &#8211; even some of the ones who look like they are yukking it up.) Gave yourself permission not to be a laugh a minute. Dropping that pressure might actually put a smile on your face.(Hear more on my <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://blogtalkradio.com/bwb">Radio Show&#8211;Blessed with Bipolar</a>)</p>
<p>2.  <strong> Do only what you want to do and only what you absolutely must do. You are free to say, &#8220;No,&#8221; to everything else.</strong> It&#8217;s your time and your weekend. You are entitled to decide what you want to do, what you don&#8217;t want to do, and what you simply cannot or will not do. There are many holiday activities that can sometimes be great fun, but not always. You may enjoy having a picnic, going to a pool, or taking a baseball or football game. It might even give you a lift to force yourself to go. But if you are going only because you are feeling some unnecessary obligation, then you are likely to come back miserable. If you do not want to go and if going is not really necessary, then respectfully tell your family and friends that you won&#8217;t be attending with them. And feel good about having the assertiveness to take care of your own mental health.</p>
<p>3.   <strong>Consider maintaining your normal routine as much as possible.</strong> Sometimes, it is good to do something new and outside of your routine &#8211; but let it be your choice. Part of the difficulty of holidays for me is the abrupt change in routine. Sometimes I enjoy that. But there are other times when I prefer to stick with the same old stuff. Let it be your choice.</p>
<p>4.   <strong>Remember that it is only 72 hours long. Tuesday will show up right on time</strong>!</p>

			<content:encoded><![CDATA[<p>During my seventh hospitalization for bipolar disorder &#8211; 18 years after I first cracked-up as a graduate student in psychology &#8211; I finally wrote a plan to keep myself out of the nuthouse. Here are some steps that you can use to write your own plan to Stop a Downward Bipolar Spiral.</p>
<p><strong>1. Identify your pattern of thoughts, feelings, and behaviors that ends in a bipolar episode.</strong></p>
<p><span id="more-3073"></span></p>
<p>We all have our own patterns. Mine starts with a frustrating event that I interpret as a problem and then exaggerate to be more serious than it is. The pattern ends with me telling myself that I am a complete failure, becoming depressed, feeling hopeless, and responding to the hopelessness with rage. When you recognize the pattern starting, you can stop it. But first, you need to know what the pattern is.</p>
<p><strong>2. Write down your pattern.</strong></p>
<p>This will give you a more clear and specific idea of your pattern and will help you recognize it as soon as it begins.</p>
<p><strong>3. Make note of the times when you have been able to avoid or stop the pattern.</strong></p>
<p>Recognizing that you have avoided potential bipolar episodes in the past (probably many times) will help you to avoid them in the future. It is a skill that you most likely have already used without even knowing it. You may have bipolar disorder, but there are many times when your mood, thoughts, and behavior are quite normal. And, sometimes, you do respond to stress quite well. What did you do at those times that enabled you to successfully avoid a bipolar meltdown?</p>
<p><strong>4. Remember that you do have some control over the pattern.</strong></p>
<p>The chemical imbalance of bipolar can powerfully affect our moods, but we can still make conscious, deliberate choices about our thoughts and behaviors. You can literally force yourself to think new thoughts and change your focus. Once when I was thinking obsessively and unproductively about something that may (or may not) have been a problem, a friend suggested that I do a crossword puzzle. I was desperate, so, in spite of my serious doubts, I gave it a try. It worked. I stopped obsessing &#8211; at least while I was doing the puzzle.</p>
<p><strong>5. Make a list of thoughts and behaviors you can use to replace your bipolar pattern thoughts, feelings, and behaviors.</strong></p>
<p>When I recognize that my pattern might be starting, I have a number of alternative thoughts and behaviors that I can use to interrupt, contradict, and stop the pattern. They include: recalling my past successes, exercise, positive self-talk, taking a walk, removing myself from stressful situations, praising God in spite of my circumstances, calling my counselor, and declaring the promises of scripture. (I roar out loud with a fist in the air, &#8220;Yes! The Creator-King of the universe is my Dad and He is right now at work for my absolute best!&#8221; Romans 8:28)</p>
<p>It&#8217;s wise to write the list down. Once your bipolar pattern thinking/feeling/acting begins it can be hard to recall the good ideas you had when you were a little more sane. Keep the list handy and pull it out as soon as the pattern begins.</p>
<p><strong>6. Give a copy of your plan to your family and/or counselor.</strong></p>
<p>In the midst of a bipolar episode, these people can pull out their copy and remind you of your own good advice. Nothing short of seroquel stops distorted thinking quite like being confronted with your own words.</p>
<p>Your plan doesn&#8217;t have to be anything at all like mine. But make a plan! Don&#8217;t leave yourself exposed to the next potential downward spiral. You can stop it! Make a plan.</p>

			<content:encoded><![CDATA[<p>Approximately 8 million American adults may be affected with bipolar disorder. People with bipolar disorder experience extreme mood swings from lows called bipolar depression to highs called bipolar mania. To further explore the impact of bipolar depression on people&#8217;s lives, the National Council for Community Behavioral Healthcare and AstraZeneca commissioned a new online survey of 2,005 patients living with bipolar disorder and 500 physicians who treat patients with bipolar disorder. Much of the findings from this survey are cause for alarm.</p>
<p>Despite being treated for bipolar disorder, some patients continue to struggle with functioning in their day-to-day lives, particularly as a result of bipolar depression.</p>
<p><span id="more-3068"></span></p>
<p>- The greatest concerns among people living with bipolar disorder are that their symptoms will have an impact on daily life, such as family, relationships, or job (73%), and that they will have long periods of depressive episodes (63%).<br />- 90% of people living with bipolar disorder said that they have difficulty managing or completing day-to-day tasks due to their bipolar depression symptoms.<br />- Approximately nine in 10 patients (89%) said they have cancelled social engagements because of bipolar depression.<br />- When asked how they felt most of the time, only 27% of people living with bipolar disorder said they felt well-balanced and just 23% felt their depression was well-controlled. <br />- The majority of people living with bipolar disorder agreed that bipolar depression causes difficulty in their ability to manage housework (73%), run errands (59%), groom themselves (53%), and pay bills (49%).</p>
<p>For most people with bipolar disorder, bipolar depression lasts longer and occurs more frequently than bipolar mania.</p>
<p>- 61% of people with bipolar disorder said their depressive episodes last longer than their manic episodes.<br />- 83% of physicians said their patients tend to have more depressive episodes than manic episodes, and 88% also noted that their patients&#8217; depressive episodes last longer than their manic episodes.</p>
<p>The survey revealed some communication challenges that people with bipolar disorder have with their physicians.</p>
<p>- 41% of people living with bipolar disorder said they wished they could be more open with their physicians about their depressive symptoms.<br />- 72% of physicians agreed that many of their patients don&#8217;t know how to accurately describe their bipolar depression symptoms.<br />- Almost half of people living with bipolar disorder (49%) said they don&#8217;t know how to accurately describe their symptoms of bipolar depression to their health care provider.</p>
<p>A barrier that some people with bipolar disorder have faced is previous misdiagnosis and improper treatment resulting from the misdiagnosis.</p>
<p>- 42% of people living with bipolar disorder said they have been misdiagnosed with and improperly treated for another mental health condition.<br />- One-third of patients (31%) said more than five years had elapsed from the time they first sought advice from a medical professional to the time they were diagnosed with bipolar disorder. Furthermore, people living with bipolar disorder listed the following as a few of the reasons for their delayed diagnoses: &#8220;I thought I was just depressed&#8221; (67%) and &#8220;I did not know that people with bipolar disorder experienced depression as much as I did&#8221; (45%).<br />- More patients with bipolar disorder (40%) said their depressive symptoms first motivated them to seek medical advice, rather than both depressive and manic symptoms (35%) and manic symptoms alone (12%). Additionally, 80% of physicians said that depressive symptoms first prompted their patients with bipolar disorder to seek medical attention.</p>
<p>Most people living with bipolar disorder feel that their medication does not fully treat all their depressive symptoms. However, many are hesitant to change their medication.</p>
<p>- 65% of people living with bipolar disorder agreed that they don&#8217;t feel like their medication fully treats their feelings of bipolar depression.<br />- 89% of people living with bipolar disorder surveyed agreed with the statement, &#8220;I would like to feel better than I do.&#8221;<br /> &#8211; 70% of physicians said their patients with bipolar disorder would prefer to stay on a familiar medication even if it does not completely resolve their symptoms than switch to an unfamiliar medication.<br />- 53% of people living with bipolar disorder agreed that they would rather feel &#8220;less than perfect&#8221; than go through another medication switch.</p>

			<content:encoded><![CDATA[<p>I keep thinking that there must be some mad connection between my Bipolar Disorder and the fact that the only sports team I consider worthy of my mania has just set a North American record for futility.</p>
<p>My Pittsburgh Pirates have not had a winning season in 17 years. Not since the blasted 1992 Atlanta wretched Braves came from two runs down in the bottom of the ninth inning of game seven of the National League Playoffs &#8211; to keep the Pirates out of the World Series – has my team had a season in which it won more than it lost. 1992?! George W. Bush&#8217;s father was President and the Pirates left-fielder was a skinny kid named Barry Bonds. Have you seen Barry lately? Skinny was a long time ago.</p>

<p> Never has there been a professional team on this entire continent – in any sport – that has had as many consecutive losing seasons as my Pirates. Never . . . in the history . . . of . . . the Continent! And my bipolar brain will not even consider giving up on them.</p>
<p></p>

<p> Is there something wrong with me – or, for some reason, does it make perfect sense that a manic-depressive would love such a loser?</p>
<p></p>
</p>
<p> Most baseball fans whom I know have eternally disavowed the Pittsburgh Pirates, delighting with ridicule for my team. I, on the other hand, fully believe that they will not only soon win more than they lose, but that they are already on their way to making the playoffs in 2011 or 2012.</p>
<p></p>
</p>
<p> In some goof-ball way, bipolar has turned me into one of those never-give-up, pain-in-the-butt, blissful idiot optimists. (I call it being <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://www.bipolarman.org">Blessed with Bipolar</a>) Doesn&#8217;t it seem that it should be just the opposite? Having bipolar means that I am prone to depression so severe that it has left me locked in a psych ward. Doesn&#8217;t that dictate that I would almost always take the most negative, glass-half-empty outlook on nearly everything – especially a baseball team that I have watched lose unceasingly for 17 years?! They have continuously blundered, not only on the field, but in scouting<strong>, </strong>player development, free agency, trades, game strategy, and every other way that a professional baseball organization can screw-up. I have bipolar disorder! Watching this Titanic should be enough &#8211; all by itself &#8211; to cost me a month in the nuthouse. </p>
<p></p>

<p> I should be the most negative, hostile, ex-fan the Pittsburgh Pirates have ever had. But my deep and agitated states of crashing depression have had an alternate effect. I have learned that no matter how severe my episodes of depression have been, they have always ended. Each and every one was followed by a period of at least normal good feeling; if not real joy and enthusiasm. And when the depressed mood ended, my view of my circumstances was transformed – without the least change in my circumstances.</p>
<p></p>

<p> It may sound crazy, but in spite of the fact that my depressions have always ended, it took long years and much gloom, despair, and agony for me to realize that I have a choice. I can choose the transformed view of my circumstances even while I am still in a bad, BAD, down, losing-Pirate mood. </p>
<p></p>
</p>
<p> The chemical imbalance of bipolar may, indeed, strongly affect my moods and emotions, but I can take control of how I think about my life. I can look at my difficult circumstances, choose to see the best possibilities, and tell myself that it is good. I can look at my $40,000 in school loan debt that I cannot currently pay and say, &#8220;I&#8217;m a damn broke, dead-beat bum.&#8221; Or I can look at that $40,000 school loan debt and thank God for my tremendous education. There are, honestly, legitimate reasons for both perspectives. Which one has the greater potential to empower me to succeed?</p>
<p></p>

<p> And I can look at the Pirates 2009 record of 62 wins and 99 losses and throw in the towel. Or I can say, &#8220;Yes! That terrible record means they get the second pick in the draft. They&#8217;re gonna get another great prospect! And look at all the young, high-potential guys they already have – Pedro Alvarez! Andrew McCuthen! Zach Von Rosenburg! Yeah, Finally! They&#8217;re doing things the right way.&#8221; And that perspective allows me to keep enjoying my team. Otherwise, I&#8217;d either have to give-up or keep watching and be miserable.</p>
<p></p>

<p> Optimism and pessimism are both choices based upon uncertain evidence that could go either way. You cannot know for sure how things will work out. (even though the optimist and the pessimist both believe that they are &#8220;realists.&#8221;) But which attitude is more likely to lead to success? When I look at it that way, optimism is the only sane choice – even if it turns out to be wrong.</p>
<p></p>
</p>
<p> Don&#8217;t get me wrong. I&#8217;m not talking about ignoring or denying our problems. They are very real. I&#8217;m not talking about whistling past our own headstone. I&#8217;m saying, &#8220;Face the problems boldly. But choose the possibilities.&#8221; That choice can shorten, end, and even prevent depression – take it from a seven-time psych ward bipolar patient.</p>
<p></p>
</p>
<p> And remember where you heard this first: The Ex-Pitiful Pittsburgh Pirates will win the 2012 World Series!</p>

<p>But you have to remember that bipolar disorder is a real medical illness. It&#8217;s not something you can cure with willpower. Taking bipolar disorder medication is just like taking medication for high blood pressure or heart disease.Medication can be like a pair of glasses. Bipolar disorder distorts your view of things; medication may allow you to see clearly again.</p>

<p>While your diagnosis of Bipolar Disorder may leave you feeling relieved on one hand because you finally know what is wrong, but more worried on the other, know that you are on the right path. Once your doctor or therapist has diagnosed your disorder, you can gain the knowledge you need to learn to deal with your disorder. Along with suggesting some lifestyle changes, your doctor may start you on a regimen of medicine to help control your symptoms.</p>
<p>There are several medicines available to help Bipolar Disorder, but in order for them to be effective, they must be taken exactly as your doctor prescribes. Here are some of the Bipolar medications your doctor may prescribe. The first medication used to treat Bipolar Disorder, and still prescribed today, is Lithium.</p>
<p>First used in the 1950&#8242;s, Lithium was not actually approved for use in Bipolar disorder until the 1970&#8242;s. Once your doctor prescribes Lithium, it will probably take about one week to start working and may take up to three weeks before you feel the full benefits. Along with its mood stabilizing abilities, Lithium users may also experience hair loss, thyroid problems and swelling. Your doctor may be able to prescribe medicines to help with the side effects of this Bipolar medicine.</p>
<p>Another type of Bipolar medicine often prescribed to help stabilize moods includes medicines originally formulated as anticonvulsants. These include drugs such as Depakote or Tegretol. Like Lithium, these drugs may also take up to three weeks to completely control your Bipolar symptoms.</p>
<p>Reported side effects include mild stomach cramps, hair loss, and sleepiness or grogginess during the day. The higher your medication dose, the more problems you may have with daytime sleepiness. Users of these medicines often report turning to coffee to help counteract this side effect.</p>
<p>A newer class of drugs now being used to treat the manic phase of Bipolar Disorder is called the psychotropic medicines. <p>One note of caution about most Bipolar medications &#8211; they can be very dangerous to pregnant women. He or she can help you to assess the risks to you and your baby and together you can decide how to progress with your treatment.</p>

<p>The type one and type two disorders are the most common. People dealing with type one bipolar disorder often experience random psychotic episodes and mood swings. The type two disorder deals with the more severe bipolar symptoms. At one time, it was thought that teens were the age group that most often has to deal with <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://www.bipolarcentral.com/">bipolar disorder</a> but many younger and older individuals have been diagnosed with bipolar symptoms over the last few years.</p>

			<content:encoded><![CDATA[<p>Bipolar as a term is being used so broadly that it is beginning to lose some of its meaning. It is, I think, often used as a default diagnosisespecially in children and adolescents. We dont know whats going on so we call it bipolar if there is mood instability, meltdowns, rages, or if the symptoms respond to a medication that is used to treat bipolar category symptoms.</p>
<p>So, the first clinical issue is what exactly is being treated? Diagnoses tend to be reductionistic. In other words, they are used as categorical labels instead of being recognized as a somewhat limited tool for communication between clinicians. They also tend to separate patients out into us and them. I am bipolar becomes an identity rather than a description of certain behaviors/emotions that are only part of a much greater whole that we call a person. It reminds me of Alcoholics Anonymous where someone stands up and says, Hi, Im Chuck. Im an alcoholic. Now I know that this is necessary, but I always want to ask, So, what does that mean? because viewing behavior in the whole context of the persons life helps me understand who someone is much more.</p>
<p><span id="more-3021"></span></p>
<p>When I was rotating through different services in medical school, we tended to refer to patients as, the gallbladder in room 12 or the M.I. (myocardial infarction) in the emergency room. While you can get away with this more in surgery or internal medicine, this tendency to objectify patients has more treatment implications in mental health than in the other medical specialties because the treatment protocols/algorithms are much more unpredictable in terms of responses. Treatment has to be tailored to the individual. Added to that is the use of the clinicians personality as a diagnostic/ therapeutic tool.</p>
<p>That being said, it is still important to understand the diagnostic categories that are applied to this disorder. They are: Bipolar Type I, Bipolar Type II and cyclothymia. To complicate matters, there are other subtypes such as rapid cycling and mixed states.</p>
<p>Many times, there are concurrent diagnoses such as borderline personality disorder or conduct disorders in adolescents. There are patients who have a completely atypical presentation with mood instability being the primary symptom. However, they do not clearly fit into any diagnostic concept.</p>
<p>The most well known diagnosis is also called manic-depressive. This is Bipolar Type I.</p>
<p>In this form, highs alternate with periods of depression. Highs can be indistinguishable from the psychosis of schizophrenia when an individual is at the extreme end of the spectrum. Consequently, individuals can experience auditory hallucinations, paranoia, bizarre delusions, etc. After treatment, it usually becomes more apparent whether it is or is not schizophrenia, since true schizophrenia shows residual impairment in many areas.</p>
<p>Alternatively, with bipolar disorder, the pattern is somewhat predictable in that there are recognizable highs with grandiosity, heightened sexual impulses, little to no need for sleep, excessive spending and poor judgment, which is not clearly recognized by the individual. Individuals with this kind of bipolar disorder are often seen in hospitals, as their impairment is unmistakable. What I see more of in an outpatient setting is the other types of bipolar disorder.</p>
<p>Bipolar Type II is characterized by elevated states with little need for sleep, creativity, impulsiveness, heightened sexuality and poor judgment, but without the clear impairment that comes with a psychotic state. I treated one patient who had essentially been in a hypomanic state for ten years and only came for treatment when a depression finally materialized in his mid- 40s. Elevated states can be hard to identify because they can be mild and others may mistake this for doing well, especially if someone is coming out of a depression.</p>
<p>Unfortunately for individuals with this profile, an antidepressant may have a counterproductive impact. These individuals may come out of the depression, pass through normal and then go into an elevated state. The hypomanic symptoms arent caused by the antidepressants, but they are triggered and aggravated by them.</p>
<p>Cyclothymia refers to the cycling of highs and lows that do not seem as severe as what you find in Bipolar Type I or II. There is some discussion regarding whether this should be considered a personality characteristics or a true form of bipolar illness. After many years of clinical experience, I think this is definitely a form of bipolar illness. Unfortunately, it isnt recognized or treated as often as it should be because it isnt as dramatic in its presentation. However, it is very frustrating to the individual who experiences its consequences. Fortunately, it responds well to treatment.</p>
<p>I think that two of the most miserable states individuals with any type of bipolar disorder can experience are rapid cycling and mixed cycling. Rapid cycling refers to changes in mood many times throughout the day. One minute youre up and the next youre down. Mixed cycling refers to feeling up and down simultaneously. This results in a jarring juxtaposition of agitation/anxiety being experienced at the same time as deep depression. Both remain difficult to treat.</p>
<p>To add to the complexity of the bipolar spectrum, there is another symptom complex that often accompanies AD/HD and OCD. It is characterized by meltdowns, mood swings, time-limited angry episodes, over-reactivity and unpredictability. These episodes can be accompanied by physical acting out. Individuals frequently dont remember much of the episode after it is over. Although this syndrome is most often identified in children and adolescents, it is readily diagnosable in adults with proper attention and clinical experience.</p>
<p>Understandably, it carries the diagnosis of bipolar disorder because it responds to the same class of medications. It seems to me, however, that it is being used as a default diagnosis. The prognosis for individuals in this category seems to be much better and I have seen many younger patients make it to adulthood with resolution of symptoms and certainly without developing a classic bipolar presentation of any kind. Did early intervention prevent full-blown development of the disorder?</p>
<p>Personally I dont think so because adults who have never been treated before have the same symptoms and also respond to the same medications. What seems most likely to me is that there are several disorders or subtypes of disorders and our conceptual framework is not broad enough yet to identify these separately.</p>
<p>So, what is the good news? First, we are now able to image brain structure as well as brain function. We are also much more sophisticated diagnostically. This means that we are much better at identifying what brain abnormalities are driving particular symptoms and which diagnoses produce similar symptoms, but require different treatments.</p>
<p>Consequently, we can look more closely at how the brain spends energy and where problems might occur. We can compare our clinical findings with what we see in PET (positron emission tomography) scans, FMRI (functional magnetic resonance imaging and SPECT (single photon emission computerized tomography).</p>
<p>Education is broadly available. In addition to local support groups and national organizations, the Internet has substantial old_resources from general information to support groups to online chat rooms where individuals can actually discuss their symptoms/concerns with similar people. This helps normalize the experience and offers opportunities to learn what strategies have been helpful to others. It is much more socially acceptable these days to admit your diagnosis and seek treatment.</p>
<p>There are now many different medications available. We understand a fair amount about what works and why. Different combinations and more choices mean that medical treatment can be tailored to the individual. If one medication doesnt work, there is probably another or a combination of two or more that will. The newer classes of medications have better side effect profiles and therefore compliance is better.</p>
<p>From my standpoint, the single most important part of treatment is the relationship with the provider or treatment team. Due to the nature of this particular set of disorders, it is often very difficult for the individual to see him/herself realistically as the mood elevates. An individual needs feedback from someone he/she trusts and who is familiar with his/her patterns. He/she needs to be followed regularly. An ongoing, timely relationship allows for confrontation and adjustment of medications before the episode gets to the point of requiring hospitalization or does serious damage to employment or family relationships.</p>
<p>WARNING! A sleep disturbance is often the herald of a manic episode and requires rapid intervention. Having a close relationship with the patient and family often means catching the episode before it escalates. I have several patients and their families who will call me immediately if sleep becomes a problem, even if there are no other active symptoms. <p>The take away message is that the future holds much promise in diagnosis and treatment. We are on the cutting edge of technology that allows us to look at the living brain and gain a much more basic understanding of all psychiatric disorders. Tracing these disorders to the level of the genome will eventually provide for correction, if not elimination, of many of these neurologic/psychiatric disorders that have plagued us for so many years. Until then, we need to fine tune our clinical skills and continue to trust in the Lord for guidance and direction.</p>
